Globally, an average of 889 dollars per person is spent on premiums, but in India, the figure is a mere 92 dollars and falling further. This shows that lakhs of Indian families remain outside the safety net of insurance.
The result is devastating: expensive medical treatment continues to push households into debt and poverty. Insurance could have been their shield, but for decades, governments neglected this sector. Only after 2014, under the Modi government, has insurance been pushed as a national necessity, not a luxury.
Out-of-Pocket Expenses: A Silent Burden
The lack of insurance forces families to spend from their own pockets for healthcare, technically known as Out-of-Pocket Expenditure (OPEP).
In 2013-14, when Congress left office, OPEP was an alarming 64.2%. Thanks to reforms post-2014, this has come down to 40% in 2021-22. But the burden remains significant.
A 2024 report by The Print revealed that in rural India, 92% of people still finance treatment from their own pockets, often by taking loans or selling assets. In semi-urban and urban areas, 77% of families face the same struggle. In cases of prolonged illness or hospitalization, medical bills have pushed lakhs of families into a debt trap.
In fact, data from 2016-17 showed that 49% of families were ruined by healthcare expenses, while 15% slipped below the poverty line. This proves how expensive treatment was not just a financial burden but a direct cause of poverty in India.
Ayushman Bharat and Now GST 2.0: A Double Shield
The Modi government’s response to this crisis has been bold and historic. The Ayushman Bharat – Pradhan Mantri Jan Arogya Yojana, offering ₹5 lakh annual insurance per family, became the world’s largest healthcare coverage scheme, transforming access to treatment for the poor. Today, nearly 55 crore Indians, including 5 crore senior citizens, hold Ayushman Cards, which have drastically reduced medical expenses for the most vulnerable.
But the middle class and urban families were still carrying a heavy load. Recognizing this, the Union Cabinet under PM Modi took another transformative step making GST zero on life and health insurance under GST 2.0 reforms.
This move makes policies more affordable, encouraging more families to opt for coverage. It also helps existing policyholders expand their cover without paying higher premiums. In simple terms, the government is ensuring that people no longer face financial ruin because of hospital bills.
Insurance as an Engine of Economic Growth
There is also a wider economic vision behind this reform. When households are not forced to spend their savings on medical treatment, that money flows into other areas education, consumer goods, travel, housing, and lifestyle spending. This directly fuels domestic consumption, the backbone of India’s economy.

By reducing healthcare-driven financial stress, India strengthens its internal market while ensuring its people are secure. This creates a strong base for India to resist global economic pressures and respond confidently to external challenges.
